View Flooring (Maintenance) Request For Proposal Form    << View Request For Proposal Forms JOB DESCRIPTION You selected the following floor types: Concrete,Wood Please provide the total usable square footage and the location for each type of flooring. The location description should include the building number, floor number, suite number, and where within suite, if applicable. You may also want to include any special contact numbers or facility hours of operation. Each floor type includes one to many duties. Select the frequency for which you would like the duty completed, or select "Not Included" if you do not require a particular duty to be performed. Some duties have footnotes (i.e. Germicide 1).
