View Insurance Requirements Section Included in Every RFP Form

INSURANCE REQUIREMENTS - Please select the appropriate answer or check the box next to each statement that applies to this job.
  A. Worker's Compensation Insurance:
    • During the life of this contract, Contractor is required to carry Worker's Compensation with the following minimum policy limits (per individual):

  • Contractor shall require subcontractors to provide insurance for the subcontractors' employees, unless such employees are covered by a Worker's Compensation policy furnished by the General Contractor.
  • Contractor may carry other insurance that is comparable to Worker's Compensation if approved by Landlord.
  B. Comprehensive General Liability Insurance:
    • Contractor shall provide and maintain Comprehensive General Liability Insurance with the following minimum liability requirements (per person/occurrence) for Bodily Injury:

    • Contractor shall provide and maintain Comprehensive General Liability Insurance with the following minimum liability requirements (per occurrence) for Property Damage:

  • Contractor shall provide and maintain Comprehensive General Liability Insurance protection including "Products and Completed Operations" coverage.
  • It is the Contractor's responsibility to assure that all sub-contractors also have Comprehensive General Liability Insurance as required by Landlord.
  C. Automobile Liability Insurance:
    • Contractor shall provide and maintain Automobile Public Liability Insurance with the following minimum policy limits for Bodily Injury or Death (per person/occurrence):

  • Said insurance policy must provide protection for owned, non-owned and hired vehicles as well as vehicles owned by Contractor.
  • It is the Contractor's responsibility to assure that any sub-contractors also have Automobile Liability Insurance as required by Landlord.
  D. Excess (Umbrella) General Liability Insurance:
    • Contractor shall provide and maintain Excess General Liability Coverage with the following minimum Policy requirements (per occurrence):

  • It is the Contractor’s responsibility to assure that any sub-contractors also have Excess General Liability Coverage as required by Landlord.
  E. Professional Liability (Errors and Omissions) Insurance:
    • The Contractor shall provide and maintain Professional Liability Coverage with the following minimum Policy requirements (per occurrence):

  • It is the Contractor’s responsibility to assure that any sub-contractors also have Professional Liability Coverage as required by Landlord.
  F. Bonding Requirements:
    • Contractor will be required to carry a Fidelity Bond (with employee dishonesty) with the following minimum Bonding requirements:

  • Crime Insurance is required to cover the deductible on the Fidelity Bond.
  • Crime Insurance is required without a Fidelity Bond.
    • Contractor will be required to carry a Performance Bond with the following minimum requirements for percentage of contract to be bonded:
  • It is the Contractor's responsibility to assure that any sub-contractors also have bonding as required by Landlord.
    • Other bonding is required for this Contract:
  G. Certificate of Insurance:
  • Contractors are not required to provide proof of insurance in order to bid on this job but are required to provide an actual Certificate of Insurance for subject job prior to the starting of any work.
  • Successful contractor will submit a Certificate of Insurance showing policy types, liability limits and required provisions with the bid.
  • The Owner, Management Company, and Subsidiaries will be named as Additional Insured by successful bidder with the following statement in the Description of Operations section of the Certificate of Insurance. "(Building Owner) and (Management Company) and their parents, affiliates, and subsidiaries are named as additional insured's."
  • Insurance is required to have a Hold Harmless Clause for the Additional Insured’s
  • A Waiver of Subrogation clause is required. The following statement will be included in the Description of Operations section of the Certificate of Insurance. "This Policy contains a Waiver of Subrogation toward the Owner and its Agents."
  • It is the Contractor’s responsibility to assure that any sub-contractors also adhere to Certificate of Insurance requirements of Landlord.
